Osteoporotic Spinal Fractures

Vertebral compression fractures are a common consequence of osteoporosis (reduced bone density). These fractures can cause significant pain, disability and progressive spinal deformity if not appropriately managed. They are particularly common in postmenopausal women and older adults.

Symptoms and Signs

  • Sudden onset of back pain, often after a minor activity
  • Pain that worsens with movement or standing
  • Loss of height over time
  • Progressive kyphosis ("dowager's hump")
  • Multiple fractures may lead to significant deformity
  • Neurological deficit (in some cases)

Investigations

  • X-ray to assess vertebral height and fracture pattern
  • MRI to assess fracture acuity and neural compression
  • CT scan for detailed bony anatomy
  • DEXA scan to assess bone density
  • Blood tests to assess metabolic bone disease

Treatment Approach

Treatment requires management of both the fracture and the underlying bone quality.

  • Pain management and activity modification
  • Bracing for pain relief
  • Osteoporosis treatment (medications to improve bone density)
  • Vertebroplasty or kyphoplasty (minimally invasive cement augmentation)
  • Surgical stabilization (when neurological involvement or major deformity exists)
