Sciatica

Sciatica refers to pain that radiates along the course of the sciatic nerve - commonly from the lower back into the buttock and down one or both legs. It is frequently caused by lumbar disc herniation or spinal stenosis compressing a nerve root. Most cases improve with appropriate conservative treatment.

Common Causes of Sciatica

  • Lumbar disc herniation (most common)
  • Lumbar spinal stenosis
  • Spondylolisthesis
  • Piriformis syndrome
  • Spinal tumors or cysts (less common)
  • Degenerative disc or facet disease

Symptoms of Sciatica

  • Pain radiating from lower back into buttock, thigh and leg
  • Sharp, burning or shooting pain down the leg
  • Numbness or tingling in the leg or foot
  • Weakness in the leg or foot
  • Pain that worsens with sitting or prolonged standing
  • Pain that may improve with walking or lying down

Treatment Approach

Most sciatica improves with conservative management. Surgery is considered when there is progressive neurological deficit or when pain is severe and not responding to appropriate non-surgical treatment.

  • Activity modification and rest (short-term)
  • Anti-inflammatory medication and analgesics
  • Physiotherapy and stretching
  • Transforaminal epidural steroid injections
  • Endoscopic or minimally invasive discectomy
  • Open microdiscectomy (when appropriate)
